Record Your Info
Please register your kit and answer the symptoms questionnaire at celiacdx.com/kit-registration. Record your information on the swab package. Our scientists will not be able to tell whether the sample is yours if the information is not completed.
Step 2
Swab
You do not need to fast or consume gluten before testing. Carefully open the swab package. Swab the inside of both cheeks for 10 seconds.
Return the swab to its original tube and screw closed. Place the tube in the original CeliacDx box with your printed information.
Step 3
Pack & Mail
Peel the strip and seal the box closed for mailing. Using the enclosed prepaid mailing sticker on the back of the box, simply drop in the mail.
